Appointments & Consultations

  • All appointments must be scheduled in advance via our website, phone, or in-person at the clinic.
  • Patients are requested to arrive at least 10 minutes before the scheduled time. Late arrivals may result in rescheduling the appointment.
  • Consultation fees and other service charges will be communicated at the time of booking and are subject to change.

Treatment Plans and Fees

  • The proposed treatment plan is based on the initial diagnosis and is subject to change depending on the patient’s response and additional findings.
  • All fees for dental treatments are explained in advance. Payment is expected at the time of service, and any changes to the treatment plan will be communicated promptly.

Patient Responsibility

  • Patients are responsible for providing accurate and complete medical history information.
  • Following post-treatment care instructions is crucial for the success of any procedure. Failure to comply may affect the outcome of your treatment.

Cancellation and No-Show Policy

  • Cancellations or rescheduling of appointments should be made at least 24 hours in advance.
  • No-show appointments may incur a fee. Repeated cancellations or no-shows may result in limited access to future appointments.

Dental Records and X-rays

  • All dental records, including x-rays, are the property of Dr Shiv’s Multispeciality Dental Clinic & Implant Center. Copies can be provided upon request, subject to applicable fees.
  • Patients may be required to sign a consent form before sharing records with third parties.

Treatment Outcomes and Risks

  • While we strive for the best possible results, treatment outcomes can vary. Patients will be informed of the potential risks and benefits of each procedure.
